Replace a licence |
If your licence is lost, stolen, destroyed or damaged, you must apply for a replacement licence.
Notify our Industry Licensing Unit in writing, stating the:
- reason for replacement
- name and address of the licence holder
- licence number (if known) and licence type.
You also need to pay a replacement fee of $35.00.
We will send you a replacement licence within approximately two weeks. During this time, we still consider you to be licensed.
Keep a copy of your request letter for future reference.
