Property agent appointment forms
	PAMD Form 20a - Appointment of agent (letting and property management) (PDF, 211 KB) To appoint a real estate agent or resident letting agent (on-site property manager) to rent out and manage your residential property.
	PAMD Form 21a - Appointment of real estate agent (commercial and industrial) (PDF, 225 KB) To appoint a real estate agent to sell, buy, rent out or manage commercial property, land or businesses for you. Do not use this form for residential property services.
	PAMD Form 22a - Appointment of real estate agent (sales and purchase) (PDF, 217 KB) To appoint a real estate agent to buy or sell property, land or businesses for you.
	PAMD Form 23 - Reappointment of real estate agent, pastoral house or auctioneer (PDF, 186 KB) To reappoint a real estate agent, pastoral house or auctioneer to buy or sell property on your behalf for another defined period as the sole or exclusive agency. Do not use this form if the first appointment period has already finished.
	PAMD Form 24a - Appointment to act as auctioneer (PDF, 267 KB) To appoint an auctioneer to auction goods or property for you. If this form is used for a motor vehicle auction, the auctioneer should attach a schedule to this form showing the particulars of the vehicle and seller.
	PAMD Form 25 - Sole or exclusive agency notice for sale of property (pastoral house) (PDF, 226 KB) To appoint a pastoral house to buy or sell rural land, property, livestock or attached buildings for you as a sole or exclusive agency. You also need to sign form appoint to act as a pastoral house, but not before signing this one.
	PAMD Form 26 - Appointment to act as pastoral house (PDF, 239 KB) To appoint a pastoral house to buy or sell rural land, property, livestock or attached buildings for you.

Property transaction forms
	PAMD Form 27c - Selling agent's disclosure to buyer (PDF, 220 KB) For real estate agents to disclose their interests, and relationships to the buyer of property who they are selling to. This must be shown to the buyer before the buyer signs a contact.
	PAMD Form 28 - Disclosure of beneficial interest to seller (PDF, 188 KB) All property agents receiving a beneficial interest by selling a property, must use this form to declare their benefit to the buyer.
	PAMD Form 30c - Warning statement (PDF, 210 KB) New PAMD Form 30c effective 1 October 2010. Real estate agents must attach this warning statement to sales contracts. It lets buyers know their rights to a cooling-off period and recommends buyers get independent property valuations and legal advice.
	BCCM Form 14 - Information sheet (PDF, 535 KB) Information sheet for the buyer of a lot in a community titles scheme.
	PAMD Form 32a - Lawyer's certifications (PDF, 200 KB) For lawyers to cancel or shorten the normal five-day cooling-off period that buyers have when they purchase residential property in Queensland.
