General forms
	Manufactured Homes Form 1 - Home owner's information document (PDF, 292 KB) Information document for prospective residential park home owners.
	Manufactured Homes Form 2 - Site agreement (PDF, 326 KB) This form may be used by home owners and residential park operators to make a site agreement.
	Manufactured Homes Form 6 - Notice to remedy breach (PDF, 193 KB) Either party can use this form to notify the other that they have broken the site agreement and request that the problem be fixed.
	Manufactured Homes Form 10 - Information for residential parks (PDF, 114 KB) Every park owner must complete this form to give to the Office of Fair Trading prescribed information about their residential park.

Termination notices
	Manufactured Homes Form 3 - Termination notice - by home owner within 28 days of entering site agreement (PDF, 203 KB) Prospective home owners of a residential park can cancel their site agreement contract during the mandatory 28 day cooling-off period.
	Manufactured Homes Form 4 - Termination notice - by mutual agreement (PDF, 202 KB) Use this form when there is mutual agreement to end the site agreement.

Sales and transfer forms

	Manufactured Homes Form 8 - Form of assignment (transfer) (PDF, 222 KB) Use this form when the seeling home owner wants to transfer their site agreement to the buyer.
	Manufactured Homes Form 8a - Notice of variation of site agreement (PDF, 92KB) Residential park owners use this form to vary a site agreement to allow periodic variations in site rent.
	Manufactured Homes Form 9 - Selling authority (PDF, 201 KB) Home owners use this form to appoint the park owner to sell their home in a residential park.
