Licence applications
	PAMD Form 1-5 and notes - Application for individual commercial agent licence (PDF, 673 KB) Individual licence application form for commercial agents. Includes PAMD Form 1-5 notes.
	PAMD Form 1-2 and notes - Application for corporate licence (PDF, 588 KB) Corporate licence application form for real estate agents, property developers, motor dealers, auctioneers or commercial agents. Includes PAMD Form 1-2 notes.
	Mutual Recognition Form 1 - Application for mutual recognition of occupational licence (PDF, 524 KB) Application form to recognise interstate or New Zealand occupational licences for real estate agents, resident letting agents, auctioneers, property developers, motor dealers or commercial agents.
	PAMD Form 11 - Application for appointment/extension of substitute licensee (PDF, 506 KB) Application form to appoint, or to extend the appointment of, a substitute licensee under the Property Agents and Motor Dealers Act 2000.

Trust account forms
