	Business Names Act Form 1 - Register a business name (PDF, 490 KB) Apply for a registered business or trading name. You can also use this form if you want to register a new name and cancel the old one.
	Business Names Act Form 4 - Statement of change in certain particulars (PDF, 454 KB) Make changes to business or postal addresses, business ownership, nature of business or business owners' names.
	Business Names Act Form 5 - Notice of cessation of business under business name (PDF, 191 KB) Notify us if you wish to cancel a business name.
